  1. This form is completed by a doctor or a midwife indicating the confinement date of the insured.

  2. This form should be completed prior to visiting the Social Security Office. Please note that in addition this completed form, required documents must be presented when visiting the Social Security Office.

  3. A life certificate MUST be submitted in order to continue receiving the monthly payments. The Life Certificate must be signed by any person listed on the Life Certificate. This activity should be done twice a year — June and December - for pensioners living in Antigua & Barbuda.

  4. A life certificate MUST be submitted in order to continue receiving the monthly payments. The Life Certificate must be signed by any person listed on the Life Certificate. This activity should be done every quarter - March, June, September and December - for pensioners living overseas.
